When you come for your appointments, please bring the following:
- Driver’s License or a valid ID
- List of medications (if any)
- We request all patients to wear a mask and follow all COVID-19 precautions.
- In case you have been exposed to COVID-19 or are feeling symptomatic, we will be more than happy to assist in rescheduling your appointment to a later date.
